OFFICIAL CONTEST RULES
CONTEST DESCRIPTION:
The ColumbusHealthyHomes Giveaway contest (“Contest”) will begin on Saturday March 27, 2010, and end on Thursday, September 30, 2010 (“Contest Period”). One (1) grand prize winner will be awarded a $500 home improvement gift card.
HOW TO ENTER:
Listeners of WCKX-FM, WXMG-FM, and WJYD-FM who are legal residents of the U.S. and are 18 years of age and older will be able to take part in this Contest. There will be four (4) methods of Contest Entry. To participate in the Contest, listeners must do one of the following:
1. Listen to WCKX-FM, WXMG-FM, and WJYD-FM (the “Participating Stations”) every Saturday through Wednesday during the Contest Period for the Columbus Healthy Homes Tip of the Week (a sixty (60) second radio ad). Listen between the hours of 4:00 p.m. to 7:00 p.m., EST on Thursdays (during the Contest Period) for the “Columbus Healthy Homes Contest” announcement by the DJ. After hearing the on-air solicitation for callers be the designated caller at the announced phone number and provide the correct tip of the week. If the correct tip of the week is provided, the correct caller will be awarded a prize pack that includes one (1) Twenty-Five Dollar ($25) home improvement gift card and be entered into the drawing for a chance to win the grand prize.
OR
2. Visit the web sites of the Participating Stations at mycolumbuspower.com, mycolumbusmagic.com, or mycolumbusjoy.com (during the Contest Period); look for the ColumbusHealthyHomes Contest icon. Upon entering the website of one of the Participating Stations, contestants must read the Healthy Home tips, click on the correct Tip of the Week and register for a chance to win by following the on-screen instructions, including entering name, address, phone number, email address, and any optional or additional contact information, as requested for a chance to win the grand prize. Online entrants will have the option of allowing their contact information to be forwarded to the City to receive more ColumbusHealthyHomes Information or they may elect not to. One online entry per person per email address during the Contest Period.
OR
3. Call the Columbus HealthyHomes hotline at 877-245-9090 and listen to the ColumbusHealthyHomes Tip of the Week. After the beep, leave your name, phone number and address to be entered into the drawing for a chance to win the grand prize.
OR
4. Attend one of the announced community events involving Columbus HealthyHomes and submit a completed entry form for a chance to win the grand prize.
5. There will be one grand prize drawing for all entry methods.
ELIGIBILITY RESTRICTIONS:
1. Listeners of the Participating Stations who are legal residents of the U.S. and are 18 years of age and older will be able to take part in this Contest.
2. Employees of the Participating Stations, Blue Chip Broadcasting, Ltd., City of Columbus, their respective parents, subsidiaries, affiliates, general sponsors, promotional partners, other radio stations in the Columbus, Ohio metropolitan area, and members of the immediate families or households of any of the above are NOT eligible to participate or win in this Contest.
3. Grand prize winner will be required to sign and submit all requested release forms. Grand prize winner’s image and name may be used for promotional purposes.
4. Contest participants may not win more than one (1) on-air prize pack for the duration of the Contest Period.
5. Persons who have won a prize in another contest or promotion of any kind from the Participating Stations within the thirty-day (30) period immediately preceding the start date of this Contest are not eligible to participate.
PRIZES:
1. On-air prize packs will include: A Twenty-Five Dollar ($25) home improvement gift card (from Lowe’s) and an informational handwashing CD. One prize pack to be given away each week for 12 weeks throughout the Contest Period.
2. One (1) Grand Prize winner will be awarded: One Five Hundred Dollar $500 home improvement gift card (from Lowe’s) to be given away by September, 30, 2010.
3. No substitution of prize is offered and non-cash prizes may not be redeemed for cash value.
4. All Contest winners must sign and date official station release and waiver forms as requested. Failure to do so will result in winner forfeiting the prize.
5. All prizes won will be awarded in the form of gift cards. Prizes will be available for pick-up no earlier than one business day from the date the Contest winner is awarded their prize.
6. If Contest winner does not claim their prize within 7 days of winning notification they forfeit the prize.
7. Winners must provide valid identification (i.e. via driver’s license, passport or other photo I.D.) to claim their prize.
EXECUTION:
1. During the Contest Period, eligible listeners will be encouraged to participate in the Contest through one of the four (4) methods of entry as described in the How To Enter section of the rules.
2. Contestants that become the correct caller will be awarded a prize pack and qualify to be entered into the drawing for a chance to win the grand prize.
3. The grand prize drawing will take place on September 30, 2010. One (1) grand prize winner will be selected by random drawing from among all eligible contestants that qualified on the air, submitted an online entry, submitted an entry by phone, and submitted an entry during an announced community event.
4. The grand prize winner will be notified by phone.
5. One winner will be awarded one (1) $500 home improvement gift card from Lowe’s.
6. Contest winners will be instructed to come to the offices of the Participating Stations located at 350 East 1st Avenue, Suite 100, Columbus, OH 43201 to claim prize(s).
7. All decisions made by the Participating Stations and Blue Chip Broadcasting, Ltd. management regarding any aspect of this Contest are final.
GENERAL CONDITIONS:
1. Winners are responsible for all applicable taxes associated with the receipt of the prize (prizes are not transferable and may not be exchanged).
2. Any person winning over $600.00 in prizes will receive an IRS form 1099 at the end of the calendar year for tax purposes and a copy of such form will be filed with the IRS as required by law.
3. No purchase necessary. Contest is void where prohibited. Contest is subject to all applicable federal, state and local laws and regulations.
4. Odds of winning depend upon the number of eligible contestants participating during the Contest Period.
5. By participating in the Contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this Contest, in any media now known or hereafter devised for advertising or promotional purposes without additional compensation, and must sign a publicity release prior to acceptance of the prize. By entering this Contest, each participant agrees to comply with the Official Rules and the decisions of the Contest sponsor which shall be final and binding in all aspects relating to the Contest.
6. All Contest winners will be required to sign a liability release prior to acceptance of any prize. Pursuant to the liability release, the winner or winners will agree to hold the Participating Stations, Blue Chip Broadcasting, Ltd., any other contest parties and each of their respective affiliates, subsidiaries, agencies, officers, shareholders, directors, employees, agents and representatives of each of the foregoing, harmless against any and all claims or liability, directly or indirectly related to or arising from or in connection with contest participation or the acceptance, possession or use / misuse of awarded prize or any portion thereof, or participation in prize-related activities, including but not limited to any related travel.
7. Winners must provide valid identification (i.e. via driver’s license, passport or other photo I.D.) to claim their prize.
8. The stations retain the right to disqualify any contestant if all Contest rules are not followed, Contest void if prohibited by law.
9. So long as no participant is materially adversely affected, the station reserves the right to make changes in the Contest rules, which will become effective upon being announced on the air.
10. The Participating Stations, Blue Chip Broadcasting, Ltd., participating sponsors and their respective parent companies, subsidiaries, affiliates or their representatives, agents and employees are not responsible for the inability of any contestant to complete or continue a telephone call, facsimile transmission or internet registration due to equipment malfunction, busy lines, inadvertent disconnections, unavailable network, server, Internet Service Provider (ISP) or other connections, communication failures or human error that may occur in the transmission, receipt or processing of entries including any injury or damage to any computer arising from or relating to participation in this Contest. In addition, the Participating Stations, Blue Chip Broadcasting, Ltd., participating sponsors and their respective parent companies, subsidiaries, affiliates or their representatives, agents and employees are not responsible for lost, misdirected, mutilated, illegible or incomplete Contest entries. Once submitted, entries become the property of the Participating Stations and will not be returned.
11. By accepting the prize, the winner acknowledges that the Participating Stations and Blue Chip Broadcasting, Ltd. have neither made, nor are in any manner responsible or liable for, any warranty, representation or guarantee, express or implied, in fact or in law, relative to the prize or to any portion thereof, including but not limited to its quality, mechanical condition or fitness for a particular purpose.
12. The Participating Stations, Blue Chip Broadcasting, Ltd., its subsidiaries, its affiliates, participating sponsors and promotional partners will not assume any responsibility for any changes in the execution of the Contest, including; cancellation of the Contest as a result of the occurrence of events not within the control of the Participating Stations and Blue Chip Broadcasting, Ltd., its subsidiaries, affiliates, participating sponsors, promotional partners and other radio stations, such as acts of God, strikes, terrorists acts or criminal acts of third parties.
13. If for any reason, the Contest is not capable of running as planned, including without limitation, any suspected evidence of tampering or technological corruption or if any portion of the Contest is compromised by virus, bugs, worms or unauthorized human intervention, fraud, an insufficient number of qualified Contest entries, or any other causes beyond the Participating Stations and Blue Chip Broadcasting, Ltd.’s control which, in Participating Stations and Blue Chip Broadcasting, Ltd.’s sole opinion, corrupts, threatens or impairs the administration, security, fairness, integrity, or proper conduct of the Sweepstakes Participating Stations and Blue Chip Broadcasting, Ltd. reserve the right to cancel, terminate, modify or suspend the Contest at their sole discretion.
14. Copies of Contest rules are available upon request during regular business hours, Monday – Friday 9:00 a.m. to 5:00 p.m., in the offices of the Participating Stations located at 350 East 1st Ave., Suite 100, Columbus, OH 43201.
